How laser treatment works
Under local anaesthetic, a narrow strip of the ingrown nail edge is removed. The laser then specifically ablates the nail root cells (matrix) at this point, so that the nail no longer grows back at the side. This leaves only a small wound, which heals quickly from the base. Surrounding tissue and skin are spared, as the laser works very finely and precisely.

- ✦
Precise and gentle on tissue
Only the affected part is treated; the healthy nail bed is preserved.
- ✚
Germ-reducing laser energy
The laser energy reduces germs and stops bleeding, which lowers the risk of infection.
- ⏱
Minimal downtime
Many patients return to their daily routine on the same day; closed shoes are usually possible after around 2 weeks.
- ◈
Outpatient at the practice
We carry out minor procedures as outpatient procedures at our practice, without a hospital stay.
Laser treatment and the Emmert procedure compared
The Emmert procedure (page in German) is the classic method using a scalpel and stitches. For most findings, laser treatment offers the gentler overall result.
Emmert procedure
- Dressing change every 2 days
- Stitches need to be removed
- Unfit for work for about 3 weeks
- Foot elevation for about 3 days
- Nail narrowing and scarring possible
- Success rate of around 70%
Laser treatment
- No dressing change, just a daily plaster change
- No stitches to remove
- Walking possible straight away, hardly any pain
- No need to elevate the foot
- Very good cosmetic result
- Success rate of around 90%
When laser treatment makes sense
An ingrown toenail (Unguis incarnatus) usually gets worse on its own. Do these symptoms sound familiar?
- Throbbing pain at the nail edge, especially in closed shoes
- Redness, swelling or warmth of the toe
- Discharge of wound fluid or pus
- Proud flesh (granulation tissue) at the nail fold
- Recurring inflammation despite treatment
- Diabetes or circulatory problems: take particular care

Worried about the procedure? Audiovisual sedation with AR glasses.
At Dr. Homssi's practice, we use AR glasses (XREAL) for audiovisual sedation. During the laser treatment, you immerse yourself in a film, music or calming nature scenes and neither hear nor see what is happening.
- Audiovisual sedation: The AR glasses take you out of the treatment situation, visually and acoustically.
- Low pain and calming: Local anaesthetic and distraction noticeably reduce tension.
- For all age groups: Especially popular with patients who are anxious about treatment or have had negative experiences in the past.
- Companion welcome: You can bring someone you trust with you.

Frequently asked questions about laser surgery
How does laser treatment of ingrown toenails work?
At Dr. Homssi's practice in Berlin, a narrow strip of the ingrown nail edge is removed under local anaesthetic. The corresponding growth zone of the nail root is then precisely ablated with a medical laser. The laser works precisely, with little bleeding, and reduces germs. As a result, the wound heals faster, and the success rate is around 90%.
Is laser surgery on the toenail painful?
No. The procedure is carried out under local anaesthetic and is painless during treatment. Because the laser is gentler on tissue than classic scalpel procedures, discomfort after treatment is considerably less. Most patients only need a mild painkiller for 1–2 days.
How long does laser surgery on the toenail take?
The procedure itself takes about 15–25 minutes per toe. Including the pre-procedure consultation, the time for the Oberst nerve block to take effect, laser ablation and dressing, you will be at our Berlin practice for around 45–60 minutes.
How long does healing take after laser surgery?
Most patients are able to go about their daily lives again by the next day. Complete wound healing usually takes 1–2 weeks. Sport and closed shoes are possible again after about 2–3 weeks.
Will the nail grow in again after laser treatment?
In around 90% of cases, the problem is permanently resolved after targeted laser ablation of the nail growth zone (matrix cauterisation); in no more than 10% does the nail grow in again. This makes laser surgery one of the most lasting methods for recurring ingrown toenails (unguis incarnatus).
What aftercare is required after laser treatment?
The dressing must be kept dry for the first 24 hours. After that, daily antiseptic foot baths (e.g. with octenidine or povidone-iodine) and dressing changes. Wound check at our Berlin practice after 3–5 days. Complete epithelialisation usually occurs after 10–14 days.
Can I go home straight after laser surgery?
Yes. The procedure is carried out as an outpatient under local anaesthetic (Oberst nerve block). You are awake throughout and can leave the practice immediately after treatment. We recommend travelling home by taxi or public transport; you can usually drive again from the following day.
Is laser treatment also suitable for people with diabetes mellitus?
Yes. The laser method is particularly advantageous for people with diabetes: minimally invasive, with little bleeding and with an antibacterial laser effect, which is a key point in diabetic neuropathy and angiopathy. A stable metabolic condition is a prerequisite; we coordinate aftercare closely with your GP or diabetologist.
From what age is laser surgery possible?
Laser matrix cauterisation can be performed safely from adolescence (approx. 14 years). For younger children, we assess each case individually and initially prefer conservative methods such as nail braces. The method is suitable for adults of all ages.
How much does laser surgery for an ingrown toenail cost in Berlin?
Depending on the findings, laser treatment is an individual service. As a rule, local anaesthetic, laser matrix cauterisation, dressing and one follow-up check are included. If both sides of the nail or several toes are affected, we will draw up a transparent cost estimate in advance based on the German medical fee schedule (GOÄ).
